When You Qualify for Both Programs
Many Camden residents qualify for Medicare because of age or disability and for NJ FamilyCare because of income. Having both can be a real advantage, but juggling both can mean extra cards, separate rulebooks and more phone calls.
Plans built for dual eligibles, called D-SNPs, are Part C plans meant to merge those benefits so care feels like one system.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.
How a D-SNP Works
Medicare remains your main coverage and NJ FamilyCare picks up much of what is left, such as premiums and copays, depending on your level of eligibility. The D-SNP coordinates both, usually assigning someone to help you line up appointments, prescriptions and services at home.
- Doctor, hospital and drug coverage in one plan
- Often very low or no copays for those with full benefits
- Extras such as dental, vision, hearing or an allowance for over-the-counter items on many plans
- A care team to help manage chronic conditions
Fully Integrated Plans in New Jersey
New Jersey offers fully integrated plans in which the same company handles your Medicare and Medicaid benefits, including long-term services some members need to remain at home. We explain which arrangements exist for Camden County and what each one includes.
Checking Your Doctors and Medicines
Like other Part C plans, D-SNPs use networks. Before any change we confirm that your primary doctor, specialists and pharmacy participate, and that each of your prescriptions is covered.
Changing Plans
People with both Medicare and Medicaid have special chances to change plans during the year, beyond the fall enrollment period. We help you understand the rules before you use one. If you are not yet enrolled in NJ FamilyCare, our cost-help page explains programs that may apply.
